Referrals
At Paul Lavery Support, we want to make things as easy as possible for busy professionals, parents, and carers.
If you are a central team at the local authority, and you would like 1-2-1 mentoring, please complete and send over either a PRF or a PRP or you can complete the attached referral form.
If you are a school or private referral and would like 1-2-1 mentoring, please complete the referral form.
If you would like to make a referral to the Reachout Farm Project, please complete and send over the Reachout Request Form.
We strive to reply within 3 working days to discuss moving forward.

Please ensure you have an agreement from whoever authorises payment from your organisation before sending the referral document. By sending the document and agreeing on a start date, you have agreed that the funding is in place for the support.
Availability is subject to change, and any dates and times offered are only valid at the moment of communication. Only when the referring professional has agreed upon the offer is the package secured
Basic report writing is included within the hourly rate, if you require a specific format please communicate this to the mentor during the introductory phase. If court level/CP level reports are required, then an additional 1 hour will be billed per report.
Our standard mentoring rate is £35ph plus VAT and mileage (mileage charged at 45 pence per mile).
For any activity budget, this will be agreed upon by the referring professional.
The above rate is for support taking place Monday – Friday, 9 am – 6 pm.
Please contact PLS for more details if you require support outside of the above times.
Reachout farm costs are £72.50 per day in addition to the standard mentor rate. The young person will be collected at 10 am and returned home at 3 pm. (Young people will need to have weather-specific clothing as well as comfortable wellies. They will also require a packed lunch)
Payment must be made in full within 14 days of the date of invoice.
If sessions are cancelled within 48 hours, we will still charge for the session.
If a package of 6 hours a day or more is to be cancelled, we require 5 days notice.
All cheques must be written to Paul Lavery Support Ltd and sent to 5 The Crescent, Stevenage, SG2 7AL.
